ConnectCare3 Offers Navigation Services throughout PA, NJ and MD

ConnectCare3 (CC3), a local, regional and national navigation service, is located in Lititz, PA. Initially it was offered as a benefit purchased by self-insured employers throughout parts of PA, NJ and MD; it is also expanding into the private sector. This unique and voluntary service advocates for patients with a wide range of diagnoses across the entire health care continuum.
CC3 was established in October, 2008, by Sam Lombardo, CEO. As he faced the diagnosis of a brain tumor, he was without a knowledgeable advocate. Throughout his lengthy and devastating recovery, he wondered if he would have had a better outcome if he would have been better informed. As a result of this unfulfilled need and a 3 year pilot study, he formed CC3 to assist people in accessing quality health care that would assure the diagnosis was accurate, patients understood the various treatment options, and there was access to experts within their community, region or nation.
CC3 is a multi-pronged voluntary service that identifies barriers to care and connects patients to credible medical information, disease experts and centers of excellence, useful social services, and insurance information. We look to integrate clinical expertise with best external evidence and empower the patient in making the most informed decision possible about their health care. Our uniqueness is the commitment to seeking the best care options for the patient and empowering them with information. This may include preparing a list of questions for physician appointments and upon request, attending that appointment with the patient.
There are currently five registered nurses on staff. CC3 also employs two patient advocates who specialize in identifying beneficial social services and understanding insurance plan documents. We also have a medical panel serving to promote quality care.
Because we are a broad based navigation service, we are eager to communicate with community and hospital based navigators to provide seamless coordinated quality care for patients and families throughout the health care continuum in PA, NJ and MD.
